how do i get a free travel pass for chronic back pain at 19?

I’m 19 years old and have had severe chronic lower back pain for nearly 5 years now. I’ve been on and off medication and seen countless doctors and had many scans and MRI’s. But I have only just heard of a disability pass that would give me free travel on public services and I would really like to know how I can avail of it in Ireland.
